OUTPATIENT PHYSICAL/
OCCUPATIONAL THERAPY APPOINTMENTS:
The Outpatient Rehab Department
is open Monday through Friday, 7:00 a.m. until 5:30 p.m.
Initial outpatient physical therapy evaluations may be scheduled
between 7 a.m. until 4:30 p.m., as this first visit requires one full hour. Follow-up
outpatient PT appointments may be scheduled between 7 a.m. until 5 p.m. as subsequent treatment sessions typically require
one half-hour to forty-five minutes of one-on-one care with the therapist. Typically, follow-up treatments are provided three
days per week to achieve your physical therapy goals. Occasionally, follow-up treatments may be scheduled five days per week
as special conditions require.
Initial occupational therapy evaluations may be scheduled between
8:00 a.m. and 2:30 p.m., as this first visit requires one full hour. Follow up appointments typically require one half-hour
to forty-five minutes and may be scheduled between 8:00 a.m. and 3:00 p.m.
SCHEDULING:
The initial outpatient evaluation is scheduled in advance and typically lasts approximately
one hour. At the time of the evaluation, please bring the physical/ occupational therapy prescription from the doctor
and your insurance cards. Follow-up treatments may be scheduled after the initial evaluation and typically last one-half hour
to one hour, depending on the condition being treated. We will attempt to accommodate your schedule within our normal
business hours, however it is recommended to schedule follow up treatment sessions at the time of your initial evaluation
if your schedule does not allow for flexibility.
INSURANCE:
Most major insurances are accepted such as
MVP, CDPHP, Medicare, Medicaid, Blue Shield of Northeastern NY, Empire Blue Cross, Worker’s Compensation Carrier’s
and No- Fault. Any co-payment required by your insurance carrier is expected at the time of the service.
ATTIRE:
Typical attire for physical therapy includes sneakers
or comfortable rubber sole shoes and comfortable clothes that allow access to the area(s) being treated. Typically, gym pants/
sweat pants and a t-shirt are the most appropriate attire.
